What we collect
We collect only the information reasonably needed to respond to enquiries, arrange appointments and provide denture services.
How information is collected
Information may be collected through our website form, by telephone or email, and during consultations, home visits and follow-up care.
Where appropriate, information may also be provided by a family member, carer, aged-care facility or another health practitioner who is authorised to assist with your care.
Why we use it
Sharing information
Information may be shared where reasonably necessary to provide or coordinate care, communicate with an authorised family member, carer, facility or health practitioner, obtain professional or technology support, process business administration, or comply with a legal or regulatory requirement.
Service providers may include website hosting, email, form-processing, data-backup, accounting and information-technology providers. We limit disclosures to what is reasonably required for the relevant purpose.
Website technology
Our website uses Google Analytics to understand website use. Google Analytics may process technical information such as IP address, browser type, device information and website activity, and may process information outside Australia under Google's privacy terms. Our contact form uses Web3Forms to deliver enquiries by email and hCaptcha to help prevent automated submissions. Web3Forms may process technical information and website enquiry information in India and through configured infrastructure regions and service providers, which may include the United States, European Union and other countries. hCaptcha is operated from the United States and may process the technical information it handles there and in other countries in which it or its service providers operate.
You can block or delete cookies through your browser settings, although some website functions may not operate correctly.
Storage and security
We take reasonable steps to protect personal and health information from misuse, interference, loss and unauthorised access, modification or disclosure.
Information may be stored in clinical records, secure business systems, email accounts and website systems. Clinical and health records are retained in accordance with applicable legal obligations, including under the Health Records and Information Privacy Act 2002 (NSW). Current minimum periods are seven years from the last occasion a health service was provided, or until the patient reaches 25 years of age where the patient was under 18. Business records are retained only as long as reasonably necessary or otherwise required by law.
Your information
You may ask to access or correct personal or health information we hold about you. We may need to confirm your identity before releasing information, and legal exceptions may apply.
Contact us if you have a privacy question or concern. We will review the issue and respond within a reasonable period.
